Referral Process
Step 1: Submit Referral
Provider submits a referral request with relevant clinical details.
Step 2: Intake & Scheduling
Patient completes intake and is scheduled for individual education sessions.
Step 3: Education Delivery
Services are delivered virtually or as designated, using evidence-based education frameworks.
Step 4: Follow-Up / Reporting (if applicable)
Progress updates or summary communication may be provided to the referring provider upon request.
Educational services do not replace individualized medical care.
Participation in education programs does not establish a provider–patient relationship for medical diagnosis or treatment decisions.
