
Group programs are clinician-led, interactive education experiences designed to support Adults living with Type 2 Diabetes through structured monthly topics, peer learning and practical skill-building. Sessions focus on evidence-based diabetes education, real-world application and culturally responsive care. Programs are offered in both virtual and live formats, depending on enrollment and setting.

Virtual Group Programs
Virtual group education sessions are conducted via SimplePractice and Zoom and are designed for interactive, small-group learning. Each virtual group is limited to a maximum of 15 participants to support meaningful discussion, education and engagement.
Virtual programs are available:
- Monthly, topic-based sessions
- Separate groups in English and Haitian Kreyòl
- Live, educator-led sessions with opportunities for questions and discussion
Participants must have reliable internet access and a compatible device.
Live (On-Site) Group Programs
Live group education programs are offered through partnering medical practices, clinics and community organizations. These programs are hosted on-site at the provider’s office or organization and are designed collaboratively to meet patient and population needs.

Language-Specific Programming
Group education sessions are offered separately in English and Haitian Kreyòl to support clarity, comfort and culturally responsive education.
How Virtual Group Programs Are Delivered
Virtual group education sessions are hosted via Zoom and managed through SimplePractice, a secure, HIPAA-compliant platform used for registration, communication and documentation.
Zoom is used solely for virtual educational delivery. Participation in group education does not establish a patient-provider relationship and does not replace individualized medical or nutrition care.
All virtual groups are capped at 15 participants to support meaningful discussion and engagement.
