Physician Referral – Individual Services

Jodi Warshafsky, MS, RD, CDCES, LDN provides individualized, evidence-based nutrition and diabetes education for patients requiring personalized clinical support. Services are designed to complement medical care and support informed decision-making across the continuum of cardiometabolic health.

  • Diabetes self-management education and support (DSMES)
  • Cardiometabolic risk reduction education
  • Weight management and GLP-1–related nutrition education (within scope of practice)

  • Coordination with referring providers when appropriate

Referral Process

Step 1: Submit Referral

Provider submits a referral request with relevant clinical details.

Step 2: Intake & Scheduling

Patient completes intake and is scheduled for individual education sessions.

Step 3: Education Delivery

Services are delivered virtually or as designated, using evidence-based education frameworks.

Step 4: Follow-Up / Reporting (if applicable)

Progress updates or summary communication may be provided to the referring provider upon request.
